ZenPet, LLC provides trusted, quality at-home pet care services and select home care services. This
agreement is a binding contract between ZenPet, LLC and the client. For the purpose of this contract, “ZenPet, LLC” refers to the Company. The term “client” refers to the pet owner and/or homeowner who is engaging the services of ZenPet, LLC. The term “pet(s)” refers to any animals(s) under the care of ZenPet, LLC.
According to the terms set forth herein, I
allow ZenPet, LLC to take care of the pet(s) identified below on the following dates.

Waiver of Release
As the Client, I acknowledge that my pet(s) may become ill or injured while in the care of ZenPet, LLC. I agree to release and hold harmless any and all liability from ZenPet, LLC should my pet(s) become lost, ill or injured, or should my home suffer any damage NOT related to any services provide by, or on the part, of the ZenPet, LLC.
I, (Client name), hereby grant permission to ZenPet, LLC to act on my behalf , and in my pet’s best interest, should my pet(s) require veterinary care if it becomes necessary for the health and well-being of my pet(s) while in ZenPet, LLC’s care. I further agree to pay for any and all costs associated with veterinary or animal health services needed while in ZenPet, LLC’s care. Further, I attest that my pet(s) is up-to-date on all vaccinations and does not suffer from any life-threatening or contagious conditions. Understanding the risks stated above, I willfully enter into this contract, including the release of ZenPet, LLC from any and all liability. This contract is full and complete, and I hereby agree to the terms.
